Office Location: My office is located in downtown Evanston at 1609 Sherman Avenue, Suite 310. The building entrance is next to Coldstone Creamery on Sherman.
Insurance: I am an in-network provider with Blue Cross Blue Shield PPO and Aetna Student Health (insurer of Northwestern University students/faculty). I also file claims for patients whose insurance is out-of-network.
